1. On the prescription form in the form N 107 / u-NP "Special prescription form for a narcotic drug or psychotropic substance" (hereinafter referred to as the prescription form), narcotic drugs or psychotropic substances included in List II of the List of narcotic drugs, psychotropic substances and their precursors are prescribed subject to control in the Russian Federation, approved (Collected Legislation of the Russian Federation, 1998, N 27, Art. 3198; 2004, N 8, Art. 663; N 47, Art. 4666; 2006, N 29, Art. 3253; 2007, N 28, item 3439; 2009, N 26, item 3183; N 52, item 6572; 2010, N 3, item 314; N 17, item 2100; N 24, item 3035; N 28, item 3703; N 31, item 4271; N 45, item 5864; N 50, item 6696, 6720; 2011, N 10, item 1390; N 12, item 1635, N 29, item 4466, 4473 ; N 42, item 5921; N 51, item 7534; 2012, N 10, item 1232; N 11, item 1295; N 22, item 2864), duly registered in the Russian Federation as medicines for medical use (hereinafter - narcotic (psychotropic) drug generic drug), with the exception of drugs in the form of transdermal therapeutic systems, as well as drugs containing a narcotic drug in combination with an opioid receptor antagonist.

2. The prescription form is filled in by the doctor who prescribed the narcotic (psychotropic) drug, or by the paramedic (midwife), who, in the manner established by the order of the Ministry of Health and Social Development of the Russian Federation of March 23, 2012 N 252n "On approval of the procedure for imposing on the paramedic , a midwife by the head of a medical organization when organizing the provision of primary health care and emergency medical care certain functions of the attending physician for the direct provision of medical care to the patient during the period of observation and treatment, including the prescription and use of drugs, including narcotic drugs and psychotropic drugs" (registered by the Ministry of Justice of the Russian Federation on April 28, 2012 N 23971), as amended by order of the Ministry of Health of the Russian Federation of February 20, 2014 N 77n (registered by the Ministry of Justice of the Russian Federation on April 22, 2014, registration N 32062), certain functions of the attending physician in prescribing and using medicines, including narcotic (psychotropic) drugs.

3. The prescription form is filled out legibly, clearly, in ink or a ballpoint pen, or using printing devices. Corrections when filling out the prescription form are not allowed.

4. The stamp of the medical organization (indicating the full name of the medical organization, its address and telephone number) and the date of issuance of the prescription for a narcotic (psychotropic) drug is affixed to the prescription form.

5. In the lines "Patient's full name" and "Age", the full surname, first name, patronymic (last - if available) of the patient, his age (number of full years) are indicated.

6. The line "Series and number of compulsory medical insurance policy" shall indicate the number of the patient's compulsory medical insurance policy (if any).

8. In the line "Full name of the doctor (paramedic, midwife)" the full surname, first name, patronymic (last - if any) of the doctor (paramedic, midwife) who issued the prescription for a narcotic (psychotropic) drug is indicated.

9. In the "Rp:" line on Latin the name of the narcotic (psychotropic) medicinal product (international non-proprietary or chemical, or in their absence, the trade name), its dosage, quantity and method of administration are indicated.

10. On one prescription form, one name of a narcotic (psychotropic) drug is written out.

The amount of the narcotic (psychotropic) medicinal product written out on the prescription form is indicated in words.

The method of taking a narcotic (psychotropic) drug is indicated in Russian or in Russian and the state languages of the republics that are part of the Russian Federation.

When specifying the method of taking a narcotic (psychotropic) medicinal product, it is prohibited to confine oneself to general indications, such as "Internal", "Known".

10. Has expired. - Order of the Ministry of Health of Russia dated April 21, 2016 N 254n.

10.1. When a patient is first issued a prescription for a narcotic (psychotropic) drug as part of the provision of medical care for a specific disease, such a prescription is certified:

1) the signature and personal seal of the doctor or the signature of the paramedic (midwife);

2) the signature of the head (deputy head) of the medical organization or the head (deputy head) of the structural unit of the medical organization or a person authorized by the head of the medical organization (in the absence of the head (deputy head) of the structural unit in the structural unit of the medical organization) (indicating his last name , name, patronymic (the last - if available));

3) the seal of a medical organization or a structural subdivision of a medical organization "For prescriptions".

When a prescription for a narcotic (psychotropic) drug is re-issued to a patient as part of the continuation of medical care for the relevant disease, the prescription is certified by the signature and personal seal of the doctor or the signature of the paramedic (midwife), the seal of the medical organization or the structural unit of the medical organization "For prescriptions" indicating in The top left corner of the recipe is labeled "Repeat".

11. In the line "Note of the pharmacy organization on the issue" the mark of the pharmacy organization on the issue of the narcotic (psychotropic) medicinal product is put (indicating the name, quantity of the dispensed narcotic (psychotropic) drug and the date of its issue).

The mark of the pharmacy organization on the release of a narcotic (psychotropic) medicinal product is certified by the signature of the employee of the pharmacy organization who dispensed the narcotic (psychotropic) medicinal product (indicating his last name, first name, patronymic (the latter - if any)), as well as the round seal of the pharmacy organization, in the imprint which the full name of the pharmacy organization should be identified.

ORDER
MANUFACTURING AND DISTRIBUTION OF SPECIAL PRECISION
FORMS FOR NARCOTIC DRUGS OR PSYCHOTROPIC SUBSTANCES

1. The Ministry of Health of the Russian Federation (hereinafter - the Ministry) organizes the production and distribution of prescription forms in the form N 107 / u-NP "Special prescription form for a narcotic drug or psychotropic substance" (hereinafter referred to as the prescription form).

2. Prescription forms are security printing products of level "B", made on pink paper measuring 10 cm x 15 cm, must have a series and number, and also comply with the requirements specified in Appendix No. 3 to the order of the Ministry of Finance of the Russian Federation dated February 7 2003 N 14n "On the implementation of the Decree of the Government of the Russian Federation of November 11, 2002 N 817" (registered by the Ministry of Justice of the Russian Federation on March 17, 2003 N 4271), as amended by order of the Ministry of Finance of the Russian Federation of July 11, 2005 .N 90n (registered by the Ministry of Justice of the Russian Federation on August 2, 2005 N 6860).

3. In order to organize the production and distribution of prescription forms, medical organizations that have the right to issue prescriptions for narcotic drugs or psychotropic substances included in List II of the List of narcotic drugs, psychotropic substances and their precursors subject to control in the Russian Federation, approved by a decree of the Government of the Russian Federation dated June 30, 1998 N 681 (Sobraniye zakonodatelstva Rossiyskoy Federatsii, 1998, N 27, art. 3198; 2004, N 8, art. 663; N 47, art. 4666; 2006, N 29, art. 3253; 2007, N 28, item 3439; 2009, N 26, item 3183; N 52, item 6572; 2010, N 3, item 314; N 17, item 2100; N 24, item 3035; N 28, item 3703; N 31, item 4271; N 45, item 5864; N 50, item 6696, 6720; 2011, N 10, item 1390; N 12, item 1635; N 29, item 4466, 4473 ; N 42, item 5921; N 51, item 7534; 2012, N 10, item 1232; N 11, item 1295), duly registered in the Russian Federation as medicinal products for medical use (hereinafter - m medical organization), draw up applications for prescription forms.

4. Until October 1 of the current year, applications from medical organizations for prescription forms are submitted:

1) medical organizations that are under the jurisdiction of federal executive authorities - to the relevant federal executive authorities;

2) medical organizations that are under the jurisdiction of the constituent entities of the Russian Federation, medical organizations of the municipal and private healthcare systems - to the executive authorities of the constituent entities of the Russian Federation in the field of healthcare on whose territory they are located (hereinafter referred to as the executive authorities of the constituent entities of the Russian Federation).

5. In the application of a medical organization for prescription forms, the following information is indicated:

1) information about the medical organization that submitted an application for prescription forms (full name, location address);

2) the number of prescription forms used by the medical organization as of January 1 of the previous year;

3) the number of prescription forms required by the medical organization for the next year.

The application of a medical organization for prescription forms is signed by the head of the medical organization, who is personally responsible for the validity and reliability of the specified information.

6. The federal executive authorities in charge of medical organizations (hereinafter referred to as the federal executive authorities), and the executive authorities of the constituent entities of the Russian Federation, on the basis of applications from medical organizations for prescription forms, form consolidated applications for prescription forms and, by December 1 of the current submit them to the Ministry.

7. The following information shall be indicated in the consolidated application for prescription forms:

1) information about the federal executive body (executive body of a constituent entity of the Russian Federation) that submitted a consolidated application for prescription forms (full name, location address);

2) the number of prescription forms distributed by the federal executive authority (executive authority of the subject of the Russian Federation) as of January 1 of the previous year between medical organizations;

3) the number of prescription forms required by the federal executive body (executive body of a constituent entity of the Russian Federation) for the next year for distribution among medical organizations;

4) information about the subordinate organization authorized by the federal executive authority (executive authority of the constituent entity of the Russian Federation) to receive prescription forms from the Ministry, store them and issue them to medical organizations (full name, location address, TIN / KPP, OKTM About the organization, actual delivery address).

8. On the basis of the received consolidated applications for prescription forms, the Ministry annually, before January 15 of the next year, forms and approves a plan for the distribution of prescription forms, which reflects the annual need of the Russian Federation for prescription forms (hereinafter referred to as the distribution plan of the Ministry).

9. The distribution plan of the Ministry shall include the following information:

1) serial number of the record;

2) the name of the federal executive body (the executive body of a constituent entity of the Russian Federation) that submitted the consolidated application for prescription forms;

3) the required number of prescription forms.

10. The direction of prescription forms to federal executive authorities, executive authorities of the constituent entities of the Russian Federation is carried out by the Ministry in the amount provided for by the distribution plan of the Ministry.

12. In the event of an increase in the need for prescription forms, federal executive authorities (executive authorities of the constituent entities of the Russian Federation) send consolidated applications to the Ministry for an additional number of prescription forms (with justification for the increase in the need for prescription forms), which are provided by the Ministry from the reserve stock of prescription forms.

13. Federal executive authorities (executive authorities of the constituent entities of the Russian Federation) distribute received prescription forms among medical organizations on the basis of distribution plans formed in accordance with applications from medical organizations for prescription forms.

1. 2. In an authorized organization and a medical organization that received prescription forms, registration, accounting and storage of prescription forms are organized.

Registration, accounting and storage of the reserve stock of special prescription forms for a narcotic drug or psychotropic substance of the Ministry is carried out by the federal state budgetary institution "All-Russian Center for Disaster Medicine "Protection" of the Ministry of Health of the Russian Federation (hereinafter, respectively, the reserve stock, institution" VTsMK "Protection").

The journal of registration and accounting of prescription forms of the reserve stock of the Ministry is also sealed by the signature of the director (in case of his absence, the deputy director) of the Department of Drug Provision and Regulation of the Circulation of Medical Devices of the Ministry and the seal of the Ministry.

5. The following information is indicated in the journal of registration and accounting of prescription forms:

1) serial number of the record;

2) receipt of prescription forms:

a) details and date of registration of the receipt document;

b) the total number of received prescription forms;

c) series and number of the prescription form;

d) the number of prescription forms for each series;

e) last name, first name, patronymic (the last one, if available) and signature of the responsible employee;

3) consumption of prescription forms:

a) date of issue of prescription forms;

b) series and numbers of issued prescription forms;

c) the number of issued prescription forms;

d) last name, first name, patronymic (the last one, if any) and signature of the person who received the prescription forms;

4) last name, first name, patronymic (the last one, if available) and signature of the responsible employee;

5) the rest of prescription forms.

6. The prescription form is a document of strict accountability and must be stored in special rooms, safes or in specially made cabinets upholstered in galvanized iron, with a reliable internal or padlock.

Premises, safes, cabinets in which prescription forms are stored must be locked and after completion of work sealed with the seal of an authorized organization (medical organization) or sealed.

7. In medical organizations, the stock of prescription forms should not exceed the six-month requirement.

8. It is allowed to issue to a medical worker who has the right to prescribe narcotic (psychotropic) drugs, no more than twenty prescription forms at a time.

Medical workers who issue prescriptions for narcotic (psychotropic) drugs, on the basis of the order of the head of the medical organization, are personally responsible for the safety of the received prescription forms.

9. A permanent commission is created in a medical organization, which at least once a month checks the status of registration and accounting of prescription forms, including by reconciling the entries of the registration log and accounting of prescription forms, including by reconciling the entries of the registration log and accounting of prescription forms. forms with the actual availability of prescription forms, as well as checking the storage status of prescription forms.

10. The federal executive authorities in charge of medical organizations and the executive authorities of the constituent entities of the Russian Federation in the field of healthcare exercise control over the organization of registration, accounting and storage of prescription forms in authorized organizations and medical organizations.

Checking the activities of the institution "VTsMK" Zashchita "on registration, accounting and storage of reserve stock is carried out by the Department of drug supply and regulation of the circulation of medical devices of the Ministry 2 times a year no later than the 20th day of the month following the expired six months.

10.1. When a patient is first issued a prescription for a narcotic (psychotropic) drug as part of the provision of medical care for a specific disease, such a prescription is certified:

3) the seal of a medical organization or a structural unit of a medical organization "For prescriptions".

When re-issuing a prescription to a patient for a narcotic (psychotropic) drug as part of the continuation of medical care for the relevant disease, the prescription is certified by the signature and personal seal of the doctor or the signature of the paramedic (midwife), the seal of the medical organization or the structural unit of the medical organization "For prescriptions" indicating in in the upper left corner of the recipe the inscription "Repeatedly".

The mark of the pharmacy organization on the dispensing of a narcotic (psychotropic) medicinal product is certified by the signature of the employee of the pharmacy organization who dispensed the narcotic (psychotropic) medicinal product (indicating his last name, first name, patronymic (the latter - if any)), as well as the round seal of the pharmacy organization, in the imprint which the full name of the pharmacy organization should be identified.

In the application of a medical organization for prescription forms, the following information is indicated:

1) information about the medical organization that submitted an application for prescription forms (full name, location address);

2) the number of prescription forms used by the medical organization as of January 1 of the previous year;

3) the number of prescription forms required by the medical organization for the next year.

The federal executive authorities in charge of medical organizations (hereinafter referred to as the federal executive authorities), and the executive authorities of the constituent entities of the Russian Federation, on the basis of applications from medical organizations for prescription forms, form consolidated applications for prescription forms and, by December 1 of the current year, submit them to the Ministry.
The summary application for prescription forms shall contain the following information:

4) information about the subordinate organization authorized by the federal executive authority (executive authority of the subject of the Russian Federation) to receive prescription forms from the Ministry, store them and issue them to medical organizations (full name, location address, TIN / KPP, OKTMO organization, actual address supplies).

Based on the received consolidated applications for prescription forms, the Ministry annually, before January 15 of the next year, forms and approves a plan for the distribution of prescription forms, which reflects the annual need of the Russian Federation for prescription forms (hereinafter referred to as the distribution plan of the Ministry).
The distribution plan of the Ministry shall include the following information:

1) serial number of the record;

2) the name of the federal executive body (the executive body of a constituent entity of the Russian Federation) that submitted the consolidated application for prescription forms;

3) the required number of prescription forms.

The direction of prescription forms to federal executive authorities, executive authorities of the constituent entities of the Russian Federation is carried out by the Ministry in the amount provided for by the distribution plan of the Ministry.
The Ministry, in addition to the number of prescription forms provided for by the distribution plan of the Ministry, forms a reserve stock of prescription forms in an amount not exceeding 100,000 prescription forms.
In the event of an increase in the need for prescription forms, federal executive authorities (executive authorities of the constituent entities of the Russian Federation) send consolidated applications to the Ministry for an additional number of prescription forms (with justification for the increase in the need for prescription forms), which are provided by the Ministry from the reserve stock of prescription forms.
Federal executive authorities (executive authorities of the constituent entities of the Russian Federation) distribute received prescription forms among medical organizations on the basis of distribution plans formed in accordance with applications from medical organizations for prescription forms.

Registration, accounting and storage of the reserve stock of special prescription forms for a narcotic drug or psychotropic substance of the Ministry is carried out by the Federal State Budgetary Institution “All-Russian Center for Disaster Medicine “Zashchita” of the Ministry of Health of the Russian Federation (hereinafter, respectively, the reserve stock, institution “VTsMK Zashchita”).

An employee appointed by order of the head of an authorized organization (authorized organization, institution "VTsMK "Protection") responsible for registration, storage and accounting of prescription forms (hereinafter referred to as the responsible employee), on the basis of a power of attorney issued in the prescribed manner (with the signature of the head of an authorized organization (authorized organization, institution "VTsMK "Protection"), chief accountant, certified by a round seal), receives prescription forms and maintains a journal of registration and accounting of prescription forms.

The register of registration and accounting of prescription forms must be numbered, laced, have an entry on the last page containing the number of pages, the full name of the authorized organization (medical organization, institution "VTsMK" Zashchita "), and also sealed with the signature of the head and the seal of the authorized organization (medical organization , institutions "VTsMK" Protection ").

b) the total number of received prescription forms;

c) series and number of the prescription form;

d) the number of prescription forms for each series;

e) last name, first name, patronymic (the last one, if any) and signature of the responsible employee;

3) consumption of prescription forms:

a) date of issue of prescription forms;

b) series and numbers of issued prescription forms;

c) the number of issued prescription forms;

d) last name, first name, patronymic (the last one, if any) and signature of the person who received the prescription forms;

4) last name, first name, patronymic (the last one, if available) and signature of the responsible employee;

5) the rest of prescription forms.

The prescription form is a document of strict accountability and must be stored in special rooms, safes or in specially made cabinets upholstered in galvanized iron, with a reliable internal or padlock.

Premises, safes, cabinets in which prescription forms are stored must be locked and after completion of work sealed with the seal of an authorized organization (medical organization) or sealed.

In medical organizations, the stock of prescription forms should not exceed a six-month requirement.

It is allowed to issue to a medical worker who has the right to prescribe narcotic (psychotropic) drugs, no more than twenty prescription forms at a time.

A permanent commission is created in a medical organization, which at least once a month checks the status of registration and accounting of prescription forms, including by reconciling the entries of the registration log and accounting of prescription forms, including by reconciling the entries of the registration log and accounting of prescription forms with the actual availability of prescription forms, as well as checking the storage status of prescription forms.

10.1. When a patient is issued a prescription for a narcotic (psychotropic) drug as part of the provision of medical care for a certain disease, such a prescription is certified:

2) the seal of a medical organization, or a structural unit of a medical organization "For prescriptions", which is affixed by a person authorized by the head of a medical organization, indicating the last name, first name, patronymic (the latter - if any) and affixing his personal signature.

In cases where it is necessary to issue a prescription for a narcotic (psychotropic) drug at home as part of the provision of palliative care, it is allowed to pre-certify the prescription with a stamp of a medical organization, a seal of a medical organization or a structural unit of a medical organization "For prescriptions", which is affixed by a person authorized by the head of the medical organization, indicating the surname, name, patronymic (the latter - if any) and affixing his personal signature. At the same time, in the journal of registration and accounting of prescription forms in accordance with the Procedure for registration, accounting and storage of special prescription forms for narcotic drugs or psychotropic substances, approved by this order, a note is made about its issuance for registration at home, as well as a mark certified by the signature of a doctor or the paramedic (midwife) who issued the prescription, about the fact of its execution, which can be additionally confirmed by photo and (or) video materials.

a) date of issue of prescription forms;

b) series and numbers of issued prescription forms;

c) the number of issued prescription forms, indicating in brackets the number of prescription forms issued for home processing;

6. The prescription form is a strictly accountable document. The stock of prescription forms in a medical organization should be stored in special rooms, safes or in specially made cabinets upholstered in galvanized iron, with a reliable internal or padlock.

7. In medical organizations, the stock of prescription forms should not exceed the six-month requirement.

The received forms are subject to strict accounting and are stored in special rooms, safes or secure cabinets. The stock of forms in a medical organization should not exceed a 3-month requirement.
